For Referring Providers & Office Staff
Referring a Patient
Referral Information (What to Send)
This section is intentionally placed first for medical office staff who need quick, clear instructions.
Preferred Referral Method: Email (preferred, especially for Mohs referrals with photographs). Email allows transmission of color clinical photographs, which are critical for accurate surgical planning.
Please Include the Following with All Referrals
We do not require a specific referral form. Please send:
- Patient demographics
- Full name
- Date of birth
- Contact phone number(s)
- Insurance information (if available)
- Reason for referral (must be clearly stated), for example:
- Transfer of dermatology care
- Spot of concern
- Full body skin examination
- Mohs surgery
- Excision
- Second opinion
- Original pathology report
- Please send the full pathology report (not a summary).
- The referral must state exactly which site(s) from the pathology report are being referred for surgery.
- Exact site(s) being referred
- If multiple sites are listed on pathology, specify exactly which lesion(s) require Mohs surgery.
- Clinical photograph(s)
- Color photographs of the biopsy site(s).
- Please email photographs directly to our Mohs referral email.

Customized Care Based on Referring Provider Preferences
Every referring practice operates differently, and I aim to adapt to your preferences rather than impose a rigid system. I will proactively check in to ask if you have specific preferences regarding communication, operative approach (including suture preferences), and documentation style.
Timely Access & Priority Scheduling
- Melanomas and melanoma in situ are prioritized for expedited evaluation and treatment
- Urgent or high-risk lesions can be discussed directly for earlier scheduling
- Patients are seen efficiently while maintaining surgical precision and safety
Follow-Up, Accountability, and Continuity of Care
- All referred patients are returned to your practice for ongoing care
- No-shows, cancellations, and rescheduled appointments are communicated back to the referring office
- Operative notes are clear, thorough, and promptly available
Multidisciplinary Collaboration
We work closely with and refer to other specialties when needed, including dermatology, primary care, ENT, radiation oncology, surgical oncology, medical oncology, plastics, oculoplastics, wound care centers, and Veterans Affairs systems. Complex cases require open, direct communication between physicians and office staff for coordination, and we are committed to that collaboration.
Questions or Physician-to-Physician Discussion
If you are unsure whether a referral is appropriate, or if you would like to discuss a case before sending it, please contact me directly. I value collaboration and am always available to support referring providers and their patients.
